As the CHLA/ABSC board embarks on a strategic planning exercise, your feedback is essential in shaping the future of the association.

The survey is open until July 15, 2022.

Your participation is entirely voluntary. You do not need to be a CHLA/ABSC member to participate. The results are confidential and anonymous. Your responses will be anonymized, and the summary of findings may be publicly available. It should take you approximately 20 minutes to complete the survey.

At the end of the survey, you will be directed to a second form and have the option to provide your name and email address to be considered for a draw for one-year complimentary membership to CHLA/ABSC for 2023. Your name and email will NOT be connected to your responses.
